What is Total Body Irradiation?

Unlike traditional radiation which targets a specific tumor site, TBI delivers a uniform dose of radiation to the entire body. While the idea of "total body" treatment can feel overwhelming, it is a highly controlled and precisely calculated procedure.

TBI serves three critical purposes in your treatment journey:

  • Eradicating Hidden Cells: It reaches cancer cells in "sanctuary sites"—areas like the bones or nervous system where chemotherapy may not easily penetrate.

  • Immune Suppression: It gently lowers your immune system’s activity so it doesn’t "reject" the donor stem cells, allowing the transplant to take hold (engraft).

  • Making Room: It clears out diseased bone marrow to create physical space for your new, healthy blood-forming cells to grow.

Precision Dosimetry & Planning

Every TBI plan is custom-designed. Our medical physicists use sophisticated software to ensure the radiation dose is perfectly balanced across your entire body, regardless of height or shape.

  • Tissue Compensators: We use custom-made devices to ensure a "homogenous" or even dose from head to toe.

  • Organ Shielding: We employ specialized "blocks" or "shields" (specifically for the lungs and kidneys) to protect your most sensitive organs from receiving the same dose as the rest of the body.

Flexible Positioning for Comfort

We understand that standing or remaining still for long periods can be difficult. Our center offers multiple positioning options:

  • Sitting or Standing: For those who are comfortable, we use a specially designed TBI stand with supports and handles.

  • Supine (Lying Down): For patients who are fatigued, we offer treatment while lying comfortably on a specialized stretcher.


What to Expect During Your Treatment

TBI is typically administered in "fractions"—small doses given twice a day over a period of 3 to 5 days.

  • The Environment: You will be in a large, private treatment room. While our therapists must step into a control room to activate the machine, they will be watching you constantly via closed-circuit TV and can speak with you through an intercom at any time.

  • The Sensation: You will not feel, see, or smell the radiation. It is much like having a long X-ray taken.

  • The Support: Because TBI is often done as an inpatient, your nursing and transplant teams work in lockstep with our radiation team to manage any immediate side effects, such as nausea or fatigue, right away.

Managing Side Effects

Your care team will provide a comprehensive "Comfort Kit" and medications to manage temporary side effects, which may include:

  • Mild skin redness (similar to a sun tan).

  • Temporary hair loss.

  • Nausea or dry mouth.

  • Fatigue as your body begins its recovery phase.

Your TBI isn't a standalone service; it is a bridge. At Thozhi, our radiation oncologists meet weekly with the Blood and Marrow Transplant (BMT) team to synchronize your radiation schedule perfectly with your transplant day.
